Carers Who Really Care

  • It is our pleasure to care for people who are frail and elderly, and are from Non-English speaking backgrounds.
  • Our carers are specially trained to provide services in the language of the client’s choice.
  • Our multicultural services are expanding and at present cover these community groups from:
Belarus, Bosnia & Hercegovina, Bulgaria, Croatia, Czech Republic, Hungary, Latvia, Lithuania, Macedonia, Poland, Romania, Russia, Serbia, Slovakia, Slovenia, Ukraine.

How You Can Access Our Services

Before you can access government-subsidised aged care services  (for example Home Care Package)  for the first time, you need to apply for an assessment. Using the online application is quick and easy and will only take 15 to 20 minutes to complete.

It’s the same form to apply for all types of care and support – including help at home, short-term care, and care in an aged care home. You can complete the application yourself or on behalf of a family member or friend. If you’d like to set up an ongoing representative, you can also do that here. There are three parts. The first checks you are eligible, the second captures your details, and the third confirms who the assessor should call to arrange the assessment. You will need your Medicare card and enough time to complete it.

Alternatively

Visit you Family Doctor and ask for a referral to My Aged Care. This referral will include your current medical conditions and your social situation. You can forward this referral to MyAgedCare with some help of your family members. If you do not have anybody to help you – please ring Rainbow Program on 92712026 and one of our bilingual workers will be happy to assist you.

We trust that you will be more than satisfied with our performance like many other people are. But as people say – nothing is so good that it cannot be improved.

We always welcome feedback and suggestions concerning any aspect of your care with us.  Importantly, please note the following:

  1. Our Care Agreement is written in simple English. This is your main document.  If you agree with it we will sign it during the initial assessment. It will explain the way our services will be delivered.  Please note this is a legal Agreement that allows us to claim the Government Subsidy.  We can then start working for you including the administrative preparation for the service delivery.

 

  1. Your Care Plan, Schedule of Service Delivery and Individual Budget. These important items are based on our initial consultation. They are developed in cooperation with yourself and/or your family. Copies of these documents are then provided to you for the initial approval and thereafter every time these documents change, for any reason.  After your initial approval these documents are becoming parts of the Care Agreement. Please call us, to discuss anything of concern or ask for a home visit, after receiving any documents that have been changed.
  1. Our Care Plans are designed to provide as much necessary information to our support workers, any emergency services and other people involved in your care such as physiotherapists or podiatrists. They are based on both the Aged Care Assessment Team’s assessment and our initial assessment. Importantly, they are developed in cooperation with yourself and/or your family. You can always change anything in them or request that some information be omitted or added. However, the Care Plan, which includes some general information about you, is about the care and services we will provide to you and not about any other aspects of your life.  Please call us to discuss anything or ask for a home visit if we need to change anything.
  1. Your Individual Budget will show you the amount of the Government Subsidy, amount of Income Tested Fees ( if applicable), amount of money deducted from the subsidy for a Home Care Package management and Care management and any costs of any planned service provision such us: hours of service, mileage, meals and other expenses for example incontinence aids, Registered Nurse assistance and more.

 

 

We do not charge on costs for organizing these services for you.

We do not charge exit fee when a client is leaving our service for any reason.

 

Our costs of services are calculated in the following way:

  • 10% of the Home Care Package Funds, which may consist of Government Funding and Income Tested Fees (if applicable), is deducted for Package management costs, 10% of Home Care Package Funds are deducted for Care management, subsequently 80% of Home Care Package Funds are spend on direct service delivery to our clients.
  • In dollars per fortnight it means that:

Home Care package level 1 current daily Government subsidy is $28.14, it gives you $393.96 per fortnight, $39.40 is deducted for HCP management and $39.40 is deducted for care management, both with unlimited hours of assistance, $315.16 is left for your direct service delivery.

Home Care package level 2 current daily Government subsidy is $49.49, it gives you $692.86 per fortnight, $69.30 is deducted for HCP management and $69.30 is deducted for care management, both with unlimited hours of assistance, $ 554.26 is left for your direct service delivery.

Home Care package level 3 current daily Government subsidy is $107.70, it gives you $1,507.80 per fortnight, $150.80 is deducted for HCP management and $150.80 is deducted for care management, both with unlimited hours of assistance, $1,206.20 is left for your direct service delivery.

Home Care package level 4 current daily Government subsidy is $163.27 it gives you $2285.78 per fortnight, $228.60 is deducted for HCP management and $228.60 is deducted for care management, both with unlimited hours of assistance, $1828.58 is left for your direct service delivery.

       Our basic cost per hour of services provided is: $ 64 on weekdays, $ 76 on Saturday, $ 89 on Sunday, $ 102 on public holiday.

                                 * Cancellation fee (2 unit’s price) will be charged for services cancelled on the day of delivery

Any other services, such as Physiotherapy, RN, podiatry, gardening, home maintenance is charged per job performed by our subcontractors. We do not charge for organizing these services.

 

Mileage paid to Rainbow Employees for driving you to appointments and shopping, and in some cases for driving to your place to deliver services, is paid from your individual budget.

 

  A once-off charge may be levied by Rainbow program for major    

   home modifications and/or capital items.

 

  1. Once a month you will receive an invoice requiring payment to us for your services. You may be asked to pay Daily Fees for services as advised by The Government. Concessions are available. You may also be asked to contribute towards necessary home and garden modifications, reasonably required to maintain the home and garden in a condition of functional safety and provide an adequate level of security. You may not be charged any fees, apart from Income Tested Fees, if there is enough money in your individual budget to cover all expenses associated with providing a Home Care package to you.

Please call us to discuss anything or ask for a home visit if we need to clarify anything.  Otherwise, non-payment of fees, followed by negative balance of your account, regrettably could mean a suspension of services.

 

  1. Once a month we will also send you a detailed financial report (which can be sent to you less often if you request it at the initial assessment or later during a home visit. Following such request your financial reports will be kept in computer files which can be accessed at any time. That report will show you:
  • the amount of the Government Subsidy for a calendar month
  • amount of Income Tested Fees you need to pay (if applicable)
  • amount of money deducted from the subsidy for Package management (10%) and Care management (10%)
  • any costs of services delivered such us: hours of service, mileage, meals, and other expenses for example continence aids, equipment, any subcontracted services (eg gardening, RN, Physio) and more.
  • it will also show you the amount of fees you paid and the balance of your individual account after that period.

Please note, it is essential that the Rainbow Program support workers receive these explanations to allow them to participate in a quality and any other improvement plans and assist you towards receiving the best possible outcome.  Please feel free to discuss these plans with your carer anytime.

We receive funds from the Australian Government  through  The  Department  of Health and Ageing and we are also supported by the ethnic communities we serve. To be fair to all of our clients, each is asked to contribute according to their ability.

This means everyone receives help regardless of their financial position
